Membership Information
Follow these steps to register to become a HOSA member!
THE REGISTRATION DEADLINE IS SEPTEMBER 15TH. THERE WILL BE NO EXTENSIONS.
1. Visit Online School Payments and scroll until you find "HOSA Membership Dues". Pay and print out your receipt.
2. Find the HOSA Membership Contract below. Fill and print out this document.
3. Complete the membership Google form, which is linked below. After it is completed, screenshot or print the submission page to show to your class HLC.
4. Lastly, you will need to bring a physical copy of the signed membership contract, along with a receipt of your paid membership dues to class, where your HLC will collect it.
5. Collect you member t-shirts after you turn in your membership contract and HOSA dues.
